Chain of Custody

Alliance Suppliers and LaMP Laboratories should use the LaMP (formerly known as GCLN) Chain-of-Custody form. Use of either the three-part paper or Excel form is acceptable. Use of the Excel form is encouraged. The new form (Rev 6 01/01/09) has been revised to implement the new PMF WBS fields included in Enfos E2. Additionally, the Project Information Form (PIF) is now included on separate tabs.

Alliance Suppliers should order paper LaMP Chain-of-Custody forms from their LaMP Laboratory. LaMP Laboratories and others who cannot obtain the forms from a LaMP Laboratory should order LaMP Chain-of-Custody forms by filling out the online order form.
