Chain of Custody

Alliance Suppliers and LaMP Laboratories should use the LaMP (formerly known as GCLN) Chain-of-Custody form. Use of either the three-part paper or Excel form is acceptable. Please make sure to use the current version (Rev 5 10/11/06), although existing stocks of the previous version (Rev 4 10/1/04) of the paper form should be used first.

Alliance Suppliers should order paper LaMP Chain-of-Custody forms from their LaMP Laboratory. LaMP Laboratories and others who cannot obtain the forms from a LaMP Laboratory should order LaMP Chain-of-Custody forms by filling out the online order form.
